Walk-in Appointments
You must bring all needed items to your appointment or it will not be processed. Please call Liz or Shanon with questions. Children must be 12 and under as of 12/25/2024.
Please Bring all of the following documents:
- Proof of all household members (photo ID for everyone over 18, birth certificate, social security card, or insurance card for all minor children).
- Proof of all household income (employment, unemployment, DSS, food stamps, SSI, SSD, child support).
- Proof of all household expenses (rent, utilities, car payment, insurance, and anything else that may be paid each month).
